Spotlight on the Beacon Communities: BCIN & Greater Cincinnati

COURSE DESCRIPTION: NeHC University will continue its series highlighting the ONC Beacon Community grant program by profiling the work of the Beacon Community of the Inland Northwest (BCIN) andthe Greater Cincinnati Beacon Community(GCBC).The program will begin with an overview of the Beacon Community Program by Kerri Petrin, Program Officer in the Office of the National Coordinator for Health IT, and will follow with presentations by the two Beacon Communities. Joining Kerri is Jac Davies, Director at BCIN who will discuss their focus on clinical practice transformation through a robust technology framework, clinical decision support tools and care coordination to improve care for patients with type 2 diabetes in the eastern Washington and north Idaho region. BCIN will discuss their focus on ensuring consistent, appropriate care, regardless of where it is provided and creating a comprehensive record so that every member of the care team has the information needed for clinical decision-making.
Pattie Bondurant, Director of GCBP, will cover her organization’s focus on implementing advanced health IT, health information exchange (HIE) and proven clinical interventions for pediatric asthma and adult diabetes. Their goal is to promote adherence to clinical guidelines as well as reduce preventable visits to emergency departments and re-hospitalizations. GCBC works with multiple local payers to create incentives for the development of patient-centered medical homes. They will also discuss their involvement in a national payment pilot through the Center for Medicare and Medicaid Innovation’s Comprehensive Primary Care initiative.
