Spotlight on the Beacon Communities: Southern Piedmont

COURSE DESCRIPTION: NeHC University will continue its series highlighting the ONC Beacon Community grant program by profiling the work of the Southern Piedmont Beacon Community.
The class will begin with an overview of the mission of the Beacon Communities from Kerri Petrin, Program Officer in the ONC. Kerri will be joined by Deb Aldrige from Southern Piedmont to discuss the Community’s two-pronged focus: to implement broad interventions across the entire community and to collaborate on shared interventions with its three local health departments and three health systems.
Southern Piedmont will discuss their progress in increasing health information exchange between providers, hospitals and other stakeholders during the course of care coordination, decreasing preventable hospital readmissions and inappropriate emergency department utilization, and improving overall public health.
