I am very glad to be part of this community. I am looking for some guidance in developing ACO Implementation models (Business Models, Process Models (Roles, Responsibilities, Activities, etc.), Sample Implementation Project Plan (Phases, Activities, Dependencies, typical effort / durations unvolved, deliverables, metrics, etc.), and any IT Systems to provide the capabilities needed to support the entire ACO Implementation Life Cycle.

I really appreciate if you can point me to any resources that will help organizations to implement new care / payment models based on PCMH, ACO, Episodes of Care / Bundled Payments, etc.

I found general information and some experience reports (from Pilots and Demonstration projects); but not specific information / guidance.

Any help is greatly appreciated.
