Consumer Consortium members are encouraged to submit examples of health systems, vendors, health centers, hospitals, HIEs and others that are successfully engaging patients with HIT in innovative and creative ways. 

Please complete the form below. Include your contact information, the name of the organization you are submitting and a brief description of their consumer engagement efforts. Please also include the rationale for your recommendation, stating why your submission would make a good profile of excellence in consumer engagement with HIT.
